Request for a Bus Pass for the next school year ONLY IF changing schools (example: Going from Elementary to Middle or Middle to High) or did not ride the bus this year and will ride next year.

PLEASE CHECK FOCUS app to see if you are already assigned for the school you are attending next year. DO NOT SUBMIT if already assigned to a bus and address in FOCUS is correct. If you move, you HAVE to change address with school before your submit this form! We will not assign the student to the bus if the address does not match FOCUS.
BUS PASSES are only valid for 5 days from date printed. If student does not turn the bus pass into the driver before 5 days, you will have to request a new bus pass.
